What happens if my image is not 4:3?

Whatever you choose. Crop to fill scales until the 640 x 480 box is covered and trims what hangs over, so you get the exact size with the edges gone. Fit inside scales the whole picture until it fits, so nothing is lost and one side comes out shorter than the box. Stretch forces both numbers and changes the shape of everything in the picture. The default is crop to fill, because an exact size is usually the reason somebody came.

640 x 480 is tiny. Will my photo still be usable?

For its purpose, yes. This is around 0.3 megapixels, which is small on a modern screen but perfectly adequate for the identity portals and embedded systems that still specify it - those display the picture at a fixed small size anyway. It is not enough for printing, and not enough to crop into afterwards, so keep your original.

Will a small photo be enlarged to 640 x 480?

It can, and it is worth avoiding. At 0.3 megapixels this is smaller than any camera output, so you should be reducing. If you find yourself enlarging to reach it, the source is already very small and the result will look worse than leaving it alone - which fit inside will do.
